Roles & Responsibilities
- Evaluate and develop individualized treatment plans to help residents regain function and independence
- Deliver skilled physical therapy interventions including gait training, therapeutic exercise and balance training
- Collaborate with nursing, OT, SLP and the interdisciplinary team to coordinate patient-centered care
- Document progress and outcomes clearly and timely in the resident's medical record
- Educate residents and families on home exercise programs and safe mobility strategies
- Support safe transfers and fall-prevention efforts within the skilled nursing environment
Qualifications
- Licensed Physical Therapist in California or eligible to obtain state licensure
- Current BLS/CPR certification (or willing to obtain)
- Experience in skilled nursing, long-term care or rehabilitation settings preferred
- Strong clinical assessment and manual therapy skills
- Excellent communication skills and comfort working within an interdisciplinary team
- Professional approach to documentation, compliance and resident safety
Education
- Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy, or equivalent degree
Knowledge & Skills
- Knowledge of geriatric rehabilitation, restorative nursing and mobility interventions
- Skilled in therapeutic exercise prescription and progressions
- Proficient with documentation standards for skilled nursing facilities
- Ability to perform safe patient handling and transfer techniques
- Familiarity with fall prevention strategies and adaptive equipment
- Strong clinical reasoning and outcome-driven care planning
