Roles & Responsibilities
- Evaluate residents to develop individualized, evidence-based treatment plans
- Deliver compassionate hands-on therapy and functional training to improve mobility and independence
- Collaborate with nursing, occupational therapy, speech therapy, and case management to support resident goals
- Document progress clearly and timely, ensuring regulatory and facility standards are met
- Educate residents and families on home programs and safe mobility strategies
- Participate in interdisciplinary meetings and contribute to discharge planning
Qualifications
- Current and unrestricted Physical Therapist license in California (or compact/reciprocal license where applicable)
- Clinical experience in skilled nursing, inpatient rehab, or geriatric care preferred
- Basic Life Support (BLS) certification required
- Strong interpersonal skills and a patient-centered approach
- Active clinical problem solving and demonstrated ability to work independently and as part of a team
Education
- Doctor of Physical Therapy (DPT) preferred or Bachelor/Master in Physical Therapy
Knowledge & Skills
- Skilled in gait training, therapeutic exercise, balance and fall prevention programs
- Knowledge of documentation standards for skilled nursing and reimbursement-driven care
- Comfortable using mobility devices, adaptive equipment, and assistive technologies
- Effective communicator with residents, families, and interdisciplinary teams
- Ability to prioritize caseloads and manage time in a fast-paced setting
