Roles & Responsibilities

• Evaluate residents to develop individualized, evidence-based treatment plans
• Deliver compassionate hands-on therapy and functional training to improve mobility and independence
• Collaborate with nursing, occupational therapy, speech therapy, and case management to support resident goals
• Document progress clearly and timely, ensuring regulatory and facility standards are met
• Educate residents and families on home programs and safe mobility strategies
• Participate in interdisciplinary meetings and contribute to discharge planning

Qualifications

• Current and unrestricted Physical Therapist license in California (or compact/reciprocal license where applicable)
• Clinical experience in skilled nursing, inpatient rehab, or geriatric care preferred
• Basic Life Support (BLS) certification required
• Strong interpersonal skills and a patient-centered approach
• Active clinical problem solving and demonstrated ability to work independently and as part of a team

Education

• Doctor of Physical Therapy (DPT) preferred or Bachelor/Master in Physical Therapy

Knowledge & Skills

• Skilled in gait training, therapeutic exercise, balance and fall prevention programs
• Knowledge of documentation standards for skilled nursing and reimbursement-driven care
• Comfortable using mobility devices, adaptive equipment, and assistive technologies
• Effective communicator with residents, families, and interdisciplinary teams
• Ability to prioritize caseloads and manage time in a fast-paced setting
