Roles & Responsibilities
- Assess residents' mobility, functional abilities, and treatment needs with thorough evaluations
- Develop individualized treatment plans that promote independence and meaningful recovery
- Implement therapeutic interventions such as gait training, therapeutic exercise, and balance programs
- Collaborate with nursing, OT, SLP, and case management to coordinate holistic care
- Document progress clearly and promptly in the facility EMR to support continuity of care
- Educate residents and families on home exercise programs and safe mobility strategies
- Monitor outcomes and adjust treatment plans to maximize functional gains and patient satisfaction
- Participate in team meetings and contribute clinical insight to care plans and discharge planning
Qualifications
- Active Physical Therapist license in California (required)
- Doctor of Physical Therapy (DPT) preferred or equivalent degree and clinical training
- Current BLS/CPR certification
- Experience in skilled nursing, outpatient, or acute care settings preferred
- Strong documentation skills and familiarity with EMR systems
- Excellent communication and teamwork skills; compassionate patient-centered approach
Education
- Doctor of Physical Therapy (DPT) preferred; Master or Bachelor in related field acceptable with appropriate licensure
Knowledge & Skills
- Skilled in therapeutic exercise, manual therapy basics, and gait training
- Knowledge of mobility devices, transfers, and fall prevention strategies
- Ability to perform thorough functional assessments and set measurable goals
- Competence with common modalities and adaptive equipment recommendations
- Effective patient and family education skills
- Comfort working in interdisciplinary teams and communicating with caregivers
