Roles & Responsibilities

• Assess patients’ physical function and design individualized rehabilitation plans
• Implement therapeutic exercises and mobility training to maximize independence
• Document patient progress clearly and timely in the medical record
• Collaborate with nursing, social work and restorative teams to coordinate care
• Educate patients and families on home exercise programs and safe transfer techniques
• Monitor outcomes and adjust plans to ensure measurable improvement
• Promote a safe environment and follow infection control and safety protocols

Qualifications

• Current Physical Therapist license in California (or ability to obtain prior to start)
• Basic Life Support (BLS) certification required; other certifications welcomed
• Experience in skilled nursing or post-acute care preferred
• Strong clinical reasoning, documentation skills and ability to work within an interdisciplinary team
• Excellent communication skills and a patient-centered approach

Education

• Doctor of Physical Therapy (DPT) preferred or equivalent entry-level PT degree

Knowledge & Skills

• Skilled in functional mobility assessment and restorative techniques
• Knowledge of evidence-based therapeutic exercise and manual techniques
• Familiarity with documentation standards in skilled nursing settings
• Ability to train and educate patients and caregivers effectively
• Comfortable using therapy equipment and adaptive devices
