Roles & Responsibilities

• Evaluate patients and develop individualized, evidence-based physical therapy plans
• Deliver hands-on and therapeutic exercise interventions to improve mobility and function
• Document progress clearly and communicate goals and outcomes with patients and the care team
• Collaborate with nurses, OTs, SLPs, and case managers to coordinate patient-centered care
• Educate patients and caregivers on home programs, safety, and injury prevention
• Monitor and modify treatment plans based on patient response and functional gains

Qualifications

• Current state Physical Therapist license or ability to obtain Wisconsin license prior to start
• Active basic life support (BLS) certification or ability to obtain
• Strong clinical assessment and manual therapy skills in adult rehabilitation
• Excellent communication skills and comfort working in an interdisciplinary setting
• Reliable, professional, and committed to delivering high-quality patient care

Education

• Graduation from an accredited Physical Therapy program (DPT preferred)

Knowledge & Skills

• Skilled in musculoskeletal and functional mobility assessment
• Experience with therapeutic exercise prescription and gait training
• Familiarity with documentation systems and regulatory compliance
• Ability to prioritize caseload and deliver efficient, safe care
• Patient education and discharge planning expertise
