Roles & Responsibilities

• Carry out treatment plans established by the supervising PT, delivering hands-on care that helps patients regain independence
• Provide therapeutic exercises, gait training, balance activities, and appropriate modalities to improve function
• Observe, document, and report patient performance and progress to the supervising PT to ensure excellent outcomes
• Educate patients and families on therapy goals and home exercise programs to extend care beyond sessions
• Maintain accurate documentation in compliance with state, federal, and company guidelines to protect patients and providers
• Collaborate with interdisciplinary staff to support coordinated, person-centered care

Qualifications

• Active and current Physical Therapist Assistant (PTA) license in California (required)
• Graduate of an accredited Physical Therapist Assistant program
• Previous experience in a Skilled Nursing Facility (SNF) required
• Strong clinical judgment and excellent communication skills
• Ability to follow treatment plans and provide high-quality patient care with accurate documentation

Education

• Graduate of an accredited Physical Therapist Assistant program

Knowledge & Skills

• Therapeutic exercise prescription, gait training, and balance interventions
• Use of modalities appropriate to patient needs
• Patient and family education and creation of home exercise programs
• Accurate clinical documentation and knowledge of compliance expectations
• Interdisciplinary teamwork and effective communication with nursing and therapy staff
• Ability to observe, assess, and clearly report patient progress
