Roles & Responsibilities

• Assess patients' functional abilities and create individualized OT treatment plans
• Deliver compassionate, goal-focused occupational therapy interventions to improve independence
• Document patient progress accurately and maintain timely clinical records
• Collaborate with nurses, PTs, SLPs and caregivers to coordinate holistic care
• Educate patients and families on strategies, adaptive equipment and safe discharge planning
• Adapt therapy approaches to diverse patient needs and changing caseloads

Qualifications

• Licensed Occupational Therapist (current license or eligibility for California licensure)
• SNF experience preferred but quick learners and adaptable clinicians are welcome
• Comfortable working temporary/contract assignments and traveling for placements
• Strong communication, documentation and teamwork skills
• Passionate, patient-centered approach and dedication to quality care

Education

• Degree in Occupational Therapy (OT) — required

Knowledge & Skills

• Skilled in OT evaluations, ADL training and functional goal-setting
• Knowledge of adaptive equipment, transfers and safe mobility techniques
• Proficient in clinical documentation and progress reporting
• Ability to collaborate effectively within interdisciplinary teams
• Good time management and caseload prioritization in fast-paced settings
