Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents while following plan of care
• Administer medications and treatments accurately and document thoroughly
• Perform focused assessments and escalate changes in condition to the clinical team
• Collaborate with nurses, therapists, and support staff to deliver coordinated care
• Maintain accurate and timely clinical documentation and shift reports
• Support resident dignity and independence while ensuring safety and compliance with facility policies

Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing
• Clinical experience in a long-term care or related setting preferred
• Ability to work scheduled night shifts and maintain consistent attendance
• Willingness to complete facility-required screenings, immunizations, and background checks

Education

• Graduate of an accredited LPN / Practical Nursing program; high school diploma or equivalent required

Knowledge & Skills

• Strong patient assessment and clinical observation skills
• Medication administration and basic wound care knowledge
• Good documentation practices and electronic charting comfort
• Effective communication and teamwork skills with residents, families, and staff
• Time management and prioritization during busy night shifts
