Roles & Responsibilities
- Provide skilled physical therapy services in the home setting, delivering patient-centered care for orthopedic and geriatric populations
- Perform SOC (start of care) visits and manage a caseload averaging about five visits per day
- Assess, plan, implement, and adjust treatment plans for patients post joint replacement and with complex comorbidities such as CHF and COPD
- Document thoroughly and promptly in the EMR systems (Netsmart and Cerner) to ensure continuity of care
- Collaborate closely with intake RNs, managers, and nurse aides to coordinate patient care and referrals
- Educate patients and caregivers on safe mobility, home exercise programs, and fall prevention
- Manage daily travel efficiently across the service area and communicate needs for caseload redistribution as the team grows
Qualifications
- Current Physical Therapist license in California (or eligible to practice in CA)
- Minimum 2 years of clinical PT experience (home health, orthopedic, or geriatric experience preferred)
- BLS (Basic Life Support) certification required
- Comfortable performing SOC visits and managing an independent caseload
- Proven ability to work with electronic medical records — experience with Netsmart and Cerner a plus
- Valid driver’s license and reliable transportation for travel across Tehama County
Education
- Degree in Physical Therapy (DPT or Bachelor/Master in PT as required for state licensure)
Knowledge & Skills
- Strong assessment and clinical reasoning skills for orthopedic and geriatric conditions
- Experience with post-op joint replacement rehabilitation and management of comorbidities (CHF, COPD)
- Proficient charting and documentation practices in Netsmart and Cerner
- Excellent communication and patient education skills for home-based care
- Time management and caseload prioritization for efficient home visits
- Ability to collaborate effectively with nursing and rehab team members
