Roles & Responsibilities

• Provide expert wound, ostomy and continence care for patients with HAPI, ostomies and wound VAC needs
• Assess wounds, develop individualized care plans, and implement evidence-based interventions
• Perform and document wound and ostomy procedures using WoundVision and Wound VAC equipment
• Collaborate with multidisciplinary teams to coordinate patient-centered care and follow-up
• Educate patients and families on wound/ostomy management and home care plans
• Maintain accurate, timely documentation in EPIC and communicate changes in patient condition

Qualifications

• Active RN licensure (California) — pending licenses are not accepted
• Minimum 2 years of RN experience; 1.5 years may be considered
• BLS certification required
• Wound care certification required (must be a certified credential — course completion alone not accepted)
• Wound and Ostomy certification or WCC preferred
• EPIC experience required
• First-time WOCN candidates accepted

Knowledge & Skills

• Strong knowledge of wound assessment, staging and treatment modalities
• Experience with Wound VAC systems and WoundVision documentation
• Comfortable managing ostomy care and patient education
• Skilled in electronic charting and documentation in EPIC
• Excellent communication and interdisciplinary collaboration skills
• Detail-oriented with strong clinical judgment and infection prevention practices
