Roles & Responsibilities
- Provide medical oversight and leadership for participant care across the PACE program
- Conduct history and physicals for new participants and perform event-based and periodic reassessments
- Develop individualized medical treatment plans and ensure medically necessary services are delivered
- Supervise and provide clinical direction to Primary Care Providers (PCPs) and Advanced Practice Providers (APPs)
- Oversee the Quality Assurance Performance Improvement (QAPI) program and drive clinical outcomes improvements
- Participate actively on the interdisciplinary team, advising on diagnoses, function, and treatment options
- Collaborate with nursing on participant treatment programs and educate participants and families about chronic conditions
- Develop and maintain medical policies, procedures, standards and protocols to ensure quality care
- Participate in intake and assessment decision-making and attend team and family conferences
- Serve in the provider on-call rotation as required
Qualifications
- MD or DO (Physician) degree required
- Active, unrestricted Massachusetts medical license required
- Prior clinical experience required (experience with geriatrics or PACE model strongly preferred)
- Demonstrated ability in clinical supervision, care coordination and quality improvement
- Comfort with interviewing participants and caregivers and making intake/assessment decisions
- Willingness to participate in scheduled on-call rotation
- Not accepting Locum candidates; sponsorship/visa candidates not accepted
Knowledge & Skills
- Strong knowledge of geriatrics and chronic care management
- Experience with the PACE model, care coordination and interdisciplinary team leadership
- Clinical skills in history & physical, ordering and interpreting diagnostic tests
- Quality improvement (QAPI) program management and performance oversight
- Ability to develop policies, procedures and clinical protocols
- Excellent communication skills for working with participants, families and team members
- Collaborative approach to working with nursing, social work, therapy and operations teams
- Comfort using electronic health records and documenting clinical encounters
