Roles & Responsibilities

• Provide compassionate direct resident care as a Licensed Nursing Assistant, ensuring dignity and comfort
• Assist with ADLs including bathing, dressing, mobility, and feeding with patience and respect
• Monitor and document resident condition changes and report concerns to clinical staff promptly
• Support a positive, team-focused environment by communicating clearly with nurses and other caregivers
• Follow infection control, safety, and facility policies to protect residents and staff
• Help create meaningful daily experiences for residents through friendly interaction and engagement

Qualifications

• Current LNA (Licensed Nursing Assistant) certification in good standing
• PCC experience required — must have prior experience with Personal Care Coordination or Patient Care Coordination
• Minimum clinical experience in a long-term care or skilled nursing environment preferred
• Current CPR/BLS certification (or ability to obtain prior to start)
• Ability to pass background checks and required health screenings
• Reliable, professional, and able to work every other weekend plus some holidays

Education

• High school diploma or equivalent; completed LNA training program required

Knowledge & Skills

• Strong resident-centered care skills and knowledge of ADLs
• Comfortable with basic clinical documentation and charting
• Good communication skills to collaborate with nursing staff and families
• Understanding of infection prevention, safety practices, and HIPAA privacy
• Ability to prioritize tasks in a fast-paced care environment
