Roles & Responsibilities

• Provide hands-on physical therapy evaluations and skilled interventions that help patients regain function
• Design and implement individualized treatment plans that reflect patient goals and evidence-based practice
• Track and document patient progress clearly and promptly to support clinical outcomes
• Collaborate with clinic staff and other healthcare providers to deliver coordinated care
• Educate patients and families on home exercise programs, injury prevention, and long-term wellness
• Maintain a compassionate, patient-centered approach that strengthens community health

Qualifications

• Licensed Physical Therapist (PT) in Minnesota or eligible to obtain Minnesota licensure
• Active Physical Therapist credentials and current basic life support/CPR certification as required
• Comfortable providing outpatient rehabilitation services with strong hands-on skills
• Excellent communication and interpersonal skills to work with patients of all ages
• Ability to manage documentation and billing requirements in a clinic setting

Education

• Doctor of Physical Therapy (DPT) or equivalent degree in Physical Therapy

Knowledge & Skills

• Skilled in musculoskeletal assessment and therapeutic exercise prescription
• Proficient with manual therapy techniques and functional mobility training
• Strong clinical reasoning and goal-oriented treatment planning
• Comfortable using electronic medical records and maintaining accurate documentation
• Empathetic patient coaching and education to promote long-term health
