Roles & Responsibilities
- Deliver skilled, patient-centered physical therapy services to residents in a skilled nursing facility
- Assess functional mobility and develop individualized treatment plans to meet rehab goals
- Provide hands-on therapeutic interventions and progressive exercise programs
- Document patient progress accurately and timely in the facility charting system
- Collaborate with nursing, OT, SLP, and other interdisciplinary team members to optimize patient outcomes
- Educate patients and families on home programs, safety, and fall prevention
- Flex to cover different units or caseloads as needed to support resident care
Qualifications
- Licensed Physical Therapist (PT) or Doctor of Physical Therapy (DPT); new grads are welcome
- Eligible for or willing to obtain Tennessee licensure in a timely manner
- Comfortable working in a Skilled Nursing Facility (SNF) setting; SNF experience is a plus
- Ability to float within the facility when requested
- Excellent communication skills and a collaborative mindset
Education
- Doctor of Physical Therapy (DPT) preferred; new graduates considered
Knowledge & Skills
- Strong knowledge of geriatric rehabilitation and functional mobility interventions
- Skilled in assessment, treatment planning, and progress documentation
- Familiarity with common SNF patient populations (post-acute, medically complex, fall risk)
- Ability to work effectively within an interdisciplinary team
- Time management and prioritization in a busy clinical setting
