Physical Therapist - Skilled Nursing

Candidate Pay Rate: $70 - $75 per hour

Roles & Responsibilities
- Evaluate and assess patients’ physical mobility and functional abilities to create a clear baseline
- Develop and implement individualized treatment plans focused on meaningful recovery goals
- Deliver therapeutic exercises, mobility training, and rehabilitation programs with compassion and clinical rigor
- Monitor patient progress, adjust treatment plans as needed, and document outcomes
- Maintain accurate, timely documentation to support continuity of care
- Collaborate with nursing, OT, speech therapy, and other interdisciplinary team members to deliver coordinated care
- Educate patients and families on recovery strategies and safe mobility techniques
Qualifications
- Active California Physical Therapist (PT) license required
- Current BLS certification required
- Previous SNF or rehabilitation experience preferred
- Strong clinical judgment and excellent communication skills
