Roles & Responsibilities

• Evaluate patients with impairments, functional limitations, disabilities or changes in physical function to create clear, patient-focused treatment plans
• Deliver skilled physical therapy treatments that restore function and reduce discomfort
• Provide meaningful patient education on prevention, wellness, and self-management strategies
• Document evaluations, progress, and discharge summaries accurately using EPIC EHR
• Collaborate with the clinical team and adhere to The Guide to Physical Therapy Practice and the Colorado State Practice Act
• Adjust plans of care according to patient response and insurance parameters to optimize outcomes
• Support continuity of care by communicating effectively with referring physicians and interdisciplinary team members

Qualifications

• Must hold or be eligible for a Colorado State physical therapy license
• Current CPR certification required
• EPIC EHR experience is required — confident charting and documentation skills in EPIC
• Past experience in orthopedic physical therapy preferred; new graduates and specialists who meet business needs will be considered
• Strong clinical reasoning, excellent communication skills, and a patient-centered approach

Education

• Diploma, BS or greater from an accredited program of Physical Therapy

Knowledge & Skills

• Proficient in physical therapy evaluation and treatment techniques
• Skilled in patient education, wellness, and prevention strategies
• Comfortable with documentation standards and using EPIC EHR
• Familiarity with The Guide to Physical Therapy Practice and Colorado State Practice Act
• Ability to work with insurance parameters while advocating for patient care
• Team-oriented with strong interpersonal and organizational skills
