Roles & Responsibilities
- Perform thorough assessments of residents’ physical abilities, limitations, and pain levels to guide care
- Develop and implement individualized, evidence-based treatment plans focused on mobility, strength, balance, and independence
- Deliver rehabilitative interventions and modify therapy as patients progress
- Collaborate with physicians, nursing, OT, SLP, and families to coordinate comprehensive care plans
- Monitor and document patient progress in accordance with facility, state, and federal guidelines
- Educate patients and caregivers on safe mobility techniques, home exercise programs, and equipment use
- Participate in care planning meetings and interdisciplinary conferences to advocate for patient goals
- Supervise physical therapist assistants and aides when appropriate to ensure quality care
Qualifications
- Current state license to practice as a Physical Therapist (required)
- Graduate of an accredited Physical Therapy program
- CPR certification (preferred or required depending on facility)
- Experience in a Skilled Nursing Facility (SNF) or geriatric rehab setting preferred
- Strong documentation skills and familiarity with Medicare/Medicaid requirements
- Excellent communication and interpersonal skills; comfortable working independently and on a team
- Ability to assist with transfers, standing, and mobility tasks as needed
Education
- Graduate of an accredited Physical Therapy program (entry-level degree required)
