Roles & Responsibilities

• Perform thorough assessments of residents’ physical abilities, limitations, and pain levels to guide care
• Develop and implement individualized, evidence-based treatment plans focused on mobility, strength, balance, and independence
• Deliver rehabilitative interventions and modify therapy as patients progress
• Collaborate with physicians, nursing, OT, SLP, and families to coordinate comprehensive care plans
• Monitor and document patient progress in accordance with facility, state, and federal guidelines
• Educate patients and caregivers on safe mobility techniques, home exercise programs, and equipment use
• Participate in care planning meetings and interdisciplinary conferences to advocate for patient goals
• Supervise physical therapist assistants and aides when appropriate to ensure quality care

Qualifications

• Current state license to practice as a Physical Therapist (required)
• Graduate of an accredited Physical Therapy program
• CPR certification (preferred or required depending on facility)
• Experience in a Skilled Nursing Facility (SNF) or geriatric rehab setting preferred
• Strong documentation skills and familiarity with Medicare/Medicaid requirements
• Excellent communication and interpersonal skills; comfortable working independently and on a team
• Ability to assist with transfers, standing, and mobility tasks as needed

Education

• Graduate of an accredited Physical Therapy program (entry-level degree required)
