Roles & Responsibilities

• Evaluate patients with injuries or conditions affecting mobility and functional independence
• Design individualized outpatient treatment plans focused on restoring function and return to activity
• Implement evidence-based interventions across orthopedic, sports, pediatric, neurological, and balance diagnoses
• Collaborate with an interdisciplinary team to coordinate patient care and optimize outcomes
• Educate patients and caregivers on home programs, fall prevention, and long-term mobility strategies
• Document progress and outcomes clearly and timely in patient records
• Travel between multiple points of care as needed to provide consistent, high-quality therapy

Qualifications

• Current unencumbered Oregon Physical Therapy license (required)
• BLS (Healthcare Provider) certification (required)
• Comfortable working in outpatient settings with diverse caseloads including ortho, sports, pediatrics, neuro, and balance
• Ability to travel to multiple points of care within the service area
• Strong communication and teamwork skills; patient-centered approach

Knowledge & Skills

• Outpatient physical therapy evaluation and treatment
• Skilled assessment of decreased physical mobility and restoration of functional independence
• Experience or familiarity with sports and orthopedic rehab
• Knowledge of pediatric, neurological, and stroke rehabilitation principles
• Balance and dizziness rehabilitation techniques and wheelchair seating considerations
• Neck, back and extremity care best practices
• Clear documentation and patient education skills
• Ability to collaborate across care settings
