Roles & Responsibilities

• Provide skilled evaluation and treatment for inpatient, swing bed/SNF and occasional hospital patients
• Develop individualized treatment plans that promote patient function and safe discharge
• Document patient progress and treatment accurately and in a timely manner
• Collaborate with nursing, case management and interdisciplinary teams to optimize patient outcomes
• Educate patients and families on home exercise programs and safe mobility strategies
• Use clinical judgment to modify interventions and escalate concerns when necessary
• Maintain up-to-date knowledge of treatment practices and apply evidence-based care
• Support efficient patient flow by prioritizing caseloads and coordinating care transitions

Qualifications

• Current California Physical Therapy License required
• Bachelor of Science in Physical Therapy (required)
• Passed the National Physical Therapist Examination (NPTE)
• Current BLS certification approved by the American Heart Association
• Minimum 1 year experience in acute care, swing bed/skilled nursing and/or outpatient settings
• Ability to use electronic documentation and common clinical software
• Strong interpersonal and communication skills; professional, timely documentation

Education

• Bachelor of Science in Physical Therapy

• Skilled in patient evaluation and development of individualized treatment plans
• Up-to-date knowledge of physical therapy treatment practices across care settings
• Comfortable with inpatient, swing bed/SNF and occasional hospital coverage
• Excellent analytical thinking and problem-solving under clinical pressure
• Compassionate patient care with the ability to manage diverse personalities and diagnoses
• Strong multi-tasking and time-management abilities
• Proficient computer skills for documentation and departmental communication
• Willingness to learn and adopt new treatment techniques
