Roles & Responsibilities

• Provide compassionate direct patient care to residents in the rehab/long-term care setting
• Administer medications and document care accurately and timely
• Monitor and record vital signs, changes in condition, and communicate concerns to the RN/MD
• Assist with wound care, treatments, and follow established care plans
• Support activities of daily living and encourage patient independence with dignity
• Collaborate with the interdisciplinary team to develop and implement individualized plans
• Maintain accurate charts and electronic medical records with attention to detail
• Advocate for patient safety, infection prevention, and a restorative environment

Qualifications

• Active LPN/LVN license (or ability to obtain state licensure in New Mexico)
• Current BLS (Basic Life Support) certification required
• Recent experience in long-term care, rehab, or similar clinical setting preferred
• Ability to pass background check and required health screenings/immunizations
• Strong clinical judgment, communication skills, and a team-first attitude

Education

• Graduate of an accredited Practical Nursing (LPN/LVN) program or equivalent diploma

Knowledge & Skills

• Medication administration and accurate documentation
• Assessment skills for identifying changes in patient condition
• Basic wound care and familiarity with common treatments in rehab settings
• Comfortable using electronic health records and charting systems
• Excellent communication with patients, families, and interdisciplinary teams
• Time management and the ability to prioritize in a fast-paced environment
