Roles & Responsibilities

• Assess speech, language, cognitive-communication and swallowing function for SNF residents
• Develop and implement individualized treatment plans that improve daily communication and swallowing safety
• Collaborate with interdisciplinary team members to coordinate care and progress toward meaningful goals
• Document evaluations, treatment notes and discharge summaries accurately and timely
• Educate residents and families to support carryover of communication and swallowing strategies
• Support facility initiatives for quality care and positive patient outcomes

Qualifications

• Certificate of Clinical Competence (CCC) — listed as a primary requirement
• SLP clinical background and comfort working in a skilled nursing facility (SNF) setting
• 1-3 years of relevant experience (per job posting)
• Comfort with some float coverage across units; team-player attitude
• Current credentials and ability to meet facility onboarding requirements (state licensure as applicable)

Knowledge & Skills

• Skilled in evaluation and treatment planning for speech, language, cognition and swallowing
• Strong documentation and clinical reasoning skills
• Excellent communication skills with residents, families and care teams
• Ability to adapt therapy approaches in a fast-paced SNF environment
• Familiarity with interdisciplinary care and discharge planning
