Roles & Responsibilities
- Assess speech, language, cognitive-communication and swallowing function for SNF residents
- Develop and implement individualized treatment plans that improve daily communication and swallowing safety
- Collaborate with interdisciplinary team members to coordinate care and progress toward meaningful goals
- Document evaluations, treatment notes and discharge summaries accurately and timely
- Educate residents and families to support carryover of communication and swallowing strategies
- Support facility initiatives for quality care and positive patient outcomes
Qualifications
- Certificate of Clinical Competence (CCC) — listed as a primary requirement
- SLP clinical background and comfort working in a skilled nursing facility (SNF) setting
- 1-3 years of relevant experience (per job posting)
- Comfort with some float coverage across units; team-player attitude
- Current credentials and ability to meet facility onboarding requirements (state licensure as applicable)
Knowledge & Skills
- Skilled in evaluation and treatment planning for speech, language, cognition and swallowing
- Strong documentation and clinical reasoning skills
- Excellent communication skills with residents, families and care teams
- Ability to adapt therapy approaches in a fast-paced SNF environment
- Familiarity with interdisciplinary care and discharge planning
