Roles & Responsibilities
- Conduct comprehensive speech-language evaluations to assess patient needs and create targeted plans
- Provide daily therapy treatments focused on functional communication, cognition, and swallowing as appropriate
- Develop and implement patient-centered discharge plans to support safe transitions
- Document progress accurately and timely to meet facility and regulatory requirements
- Collaborate with the interdisciplinary team to coordinate care and optimize patient outcomes
- Maintain professional standards and follow CRG policies and procedures while representing the facility
Qualifications
- Licensed or credentialed Speech-Language Pathologist (SLP) or equivalent credentials — please have documentation ready
- 1-3 years of clinical experience (job lists 1-3 years) with strong clinical reasoning
- Prior Skilled Nursing Facility (SNF) experience strongly preferred
- Comfortable meeting productivity expectations (job notes indicate a 90% productivity requirement)
- Ability to work on-site (therapist must be present in the building to receive guaranteed hours) and follow facility dress code (black scrubs)
Knowledge & Skills
- Skilled in speech-language evaluations and treatment planning
- Experience with discharge planning and transitional care in post-acute settings
- Strong documentation, time-management, and organizational skills
- Ability to collaborate effectively with nursing and therapy teams in a SNF environment
- Familiarity with productivity expectations and facility policies/procedures
