Roles & Responsibilities
- Provide high-quality occupational therapy services to residents in a skilled nursing facility
- Perform assessments, develop individualized treatment plans, and document progress
- Collaborate closely with the interdisciplinary therapy team to maximize patient outcomes
- Maintain productivity expectations and complete required documentation to meet the 90% productivity target
- Follow Continuum Rehabilitation Group policies and procedures (CRG P & P) and facility guidelines
- Be present in the building for scheduled hours to ensure continuity of care and receive guaranteed hours
- Communicate effectively with facility staff, patients, and families to support safe, person-centered care
Qualifications
- 1-3 years of occupational therapy experience (experience in Skilled Nursing Facility settings preferred)
- Current or eligible Occupational Therapy licensure to practice (ability to obtain NM licensure if not already licensed)
- Strong clinical assessment and treatment planning skills specific to adult and geriatric populations
- Comfortable working under facility policies and meeting productivity expectations
- Professional, reliable presence (dress code: black scrubs as specified by the facility)
- Ability to participate in an interview with the Regional Director of Therapy
Knowledge & Skills
- Skilled Nursing Facility (SNF) therapy experience and best practices
- Clinical documentation and progress note proficiency
- Knowledge of rehabilitation techniques and adaptive equipment
- Team collaboration, clear communication, and time management
- Familiarity with CRG policies and productivity standards
