Roles & Responsibilities

• Provide high-quality occupational therapy services to residents in a skilled nursing facility
• Perform assessments, develop individualized treatment plans, and document progress
• Collaborate closely with the interdisciplinary therapy team to maximize patient outcomes
• Maintain productivity expectations and complete required documentation to meet the 90% productivity target
• Follow Continuum Rehabilitation Group policies and procedures (CRG P & P) and facility guidelines
• Be present in the building for scheduled hours to ensure continuity of care and receive guaranteed hours
• Communicate effectively with facility staff, patients, and families to support safe, person-centered care

Qualifications

• 1-3 years of occupational therapy experience (experience in Skilled Nursing Facility settings preferred)
• Current or eligible Occupational Therapy licensure to practice (ability to obtain NM licensure if not already licensed)
• Strong clinical assessment and treatment planning skills specific to adult and geriatric populations
• Comfortable working under facility policies and meeting productivity expectations
• Professional, reliable presence (dress code: black scrubs as specified by the facility)
• Ability to participate in an interview with the Regional Director of Therapy

Knowledge & Skills

• Skilled Nursing Facility (SNF) therapy experience and best practices
• Clinical documentation and progress note proficiency
• Knowledge of rehabilitation techniques and adaptive equipment
• Team collaboration, clear communication, and time management
• Familiarity with CRG policies and productivity standards
