Roles & Responsibilities
- Assess and evaluate residents’ speech, language, cognition and swallowing needs with evidence-based tools
- Develop and implement individualized treatment plans that promote functional communication and safety
- Document progress, maintain accurate records, and meet facility productivity requirements (90%)
- Collaborate with nurses, therapists, and care teams to coordinate resident-centered care
- Educate residents, families, and staff on strategies to maximize communication and swallowing safety
- Follow Continuum Rehabilitation Group (CRG) policies and facility procedures to ensure quality and compliance
Qualifications
- 1–3 years of experience in speech-language pathology or related clinical settings (per job posting)
- Licensed or credentialed as an SLP as required by the state and facility (ensure appropriate state authorization before start)
- Comfortable working in a skilled nursing facility (SNF) environment and providing geriatric-focused care
- Able to meet productivity expectations (90%) and follow CRG policies and procedures
- Willing to follow facility dress code (black scrubs) and be present on-site to receive guaranteed hours
- Reliable, professional communication skills and a collaborative mindset
Education
- Master’s degree in Speech-Language Pathology or equivalent credential (typical requirement for SLP roles; confirm state/facility specifics)
Knowledge & Skills
- Skilled in clinical assessment and treatment planning for communication and swallowing disorders
- Strong documentation and time-management abilities to meet productivity targets
- Experience with SNF workflows and interdisciplinary team collaboration
- Resident-centered approach with good bedside manner and family education skills
- Ability to follow facility protocols and company policies consistently
