Roles & Responsibilities

• Provide skilled speech-language pathology evaluation and treatment for residents in a skilled nursing facility
• Design individualized treatment plans to improve communication, cognition, and swallowing function
• Collaborate with nurses, therapists, and facility staff to coordinate patient-centered care
• Maintain timely, accurate documentation and meet facility productivity expectations (90% productivity requirement)
• Follow Continuum Rehabilitation Group policies and facility procedures while representing the therapy team professionally (black scrubs dress code noted)
• Participate in team meetings and contribute to a positive, outcomes-focused therapy environment

Qualifications

• SLP clinical experience — listing specifies 1–3 years of experience (min)
• Experience working in a Skilled Nursing Facility (SNF) setting preferred (core skill listed)
• Ability to work on-site consistently (therapist must be present in the building to receive guaranteed hours)
• Comfort following company policies and meeting productivity expectations
• Professional demeanor and adherence to facility dress code (black scrubs)

Knowledge & Skills

• Strong clinical skills in adult speech, language, cognition, and dysphagia management relevant to SNF care
• Skilled in documentation, productivity tracking, and meeting facility benchmarks
• Collaborative interdisciplinary communication and care planning
• Familiarity with SNF workflows and regulatory/documentation requirements
• Ability to adapt treatment to resident-centered goals and safety needs
