Roles & Responsibilities
- Provide skilled speech-language pathology evaluation and treatment for residents in a skilled nursing facility
- Design individualized treatment plans to improve communication, cognition, and swallowing function
- Collaborate with nurses, therapists, and facility staff to coordinate patient-centered care
- Maintain timely, accurate documentation and meet facility productivity expectations (90% productivity requirement)
- Follow Continuum Rehabilitation Group policies and facility procedures while representing the therapy team professionally (black scrubs dress code noted)
- Participate in team meetings and contribute to a positive, outcomes-focused therapy environment
Qualifications
- SLP clinical experience — listing specifies 1–3 years of experience (min)
- Experience working in a Skilled Nursing Facility (SNF) setting preferred (core skill listed)
- Ability to work on-site consistently (therapist must be present in the building to receive guaranteed hours)
- Comfort following company policies and meeting productivity expectations
- Professional demeanor and adherence to facility dress code (black scrubs)
Knowledge & Skills
- Strong clinical skills in adult speech, language, cognition, and dysphagia management relevant to SNF care
- Skilled in documentation, productivity tracking, and meeting facility benchmarks
- Collaborative interdisciplinary communication and care planning
- Familiarity with SNF workflows and regulatory/documentation requirements
- Ability to adapt treatment to resident-centered goals and safety needs
