Roles & Responsibilities

• Conduct thorough speech-language evaluations to assess patient needs and develop plans of care
• Deliver daily treatments focused on communication, cognition, and swallowing as appropriate
• Plan and coordinate safe, timely discharges in collaboration with nursing and case management
• Document treatment sessions and progress notes accurately and on time
• Meet facility productivity expectations and follow Continuum Rehabilitation Group policies
• Participate as an engaged member of the interdisciplinary therapy team to optimize patient outcomes
• Maintain professional appearance (black scrubs per facility dress code) and be present on-site for scheduled shifts

Qualifications

• SLP credential/qualification appropriate for practice (as required by facility)
• 1-3 years of clinical experience (posting indicates minimum experience in this range)
• Prior SNF experience strongly preferred
• Comfortable working in a skilled nursing facility environment with a focus on evaluations, daily treatments, and discharges
• Willingness to follow CRG policies and meet the facility's productivity requirements
• Ability to be on-site for scheduled hours and adhere to facility dress code

Education
