Roles & Responsibilities

• Evaluate residents' functional status and develop individualized, evidence-based treatment plans
• Deliver skilled physical therapy interventions to improve mobility, strength, balance, and independence
• Collaborate with nursing and interdisciplinary team members to coordinate resident-focused care
• Document assessments, progress notes, and discharge plans in accordance with CRG policies and facility standards
• Monitor and adjust treatment plans to meet measurable goals and ensure continuity of care
• Maintain productivity expectations (90%) while prioritizing quality, resident-centered outcomes
• Participate in team meetings, family conferences, and care planning to advocate for residents

Qualifications

• Active PT license in New Mexico or eligible to obtain licensure prior to start
• Doctor of Physical Therapy (DPT) or equivalent physical therapy degree
• 1–3 years of clinical experience in skilled nursing, inpatient rehab, or related settings preferred
• Current CPR/BLS certification (or willing to obtain upon hire)
• Strong documentation skills and comfort with productivity targets and meeting regulatory requirements
• Ability to follow Continuum Rehabilitation Group (CRG) policies & procedures and work onsite as required
• Professional communication skills and a team-first attitude

Education

• Doctor of Physical Therapy (DPT) or equivalent physical therapy degree required/preferred

Knowledge & Skills

• Skilled knowledge of geriatric rehab and SNF workflows
• Proficiency in evaluating functional mobility, gait training, transfers, and therapeutic exercise
• Competence with care planning, outcome measurement, and regulatory documentation
• Excellent interpersonal skills for patient/family education and interdisciplinary collaboration
• Time management and organization to meet productivity expectations while delivering quality care
• Adaptability to work within established facility processes and a dedication to resident safety
