Shift Type: 5 days/week (schedule varies), day shifts with a 32-hour guarantee; split time between Heritage Manor West and Town and Country Health & Rehab

Candidate Pay Rate: $49 per hour

Roles & Responsibilities
- Provide direct patient care through thorough evaluations and individualized treatment plans
- Perform manual therapy and hands-on interventions to improve patient mobility and function
- Assess and assist with transfers, gait, and balance to promote safe independence
- Maintain accurate clinical documentation and ensure correct billing/reporting for your caseload
- Coordinate care with interdisciplinary team members and participate in patient-care meetings
- Develop and present in-service trainings to staff and contribute to community education efforts
- Supervise and mentor support personnel in immediate work areas
- Educate patients and families on treatment plans, goals, and home programs
Qualifications
- Bachelor's degree in Physical Therapy (required)
- Current certification with the American Physical Therapy Association (APTA)
- State Physical Therapy License for Louisiana (must be active/current)
- Able to safely lift, turn and transfer patients (able to control ~50 pounds for minimal assist transfers)
- Comfortable performing manual manipulation and physically demanding patient care (approximately 90% physical tasks)
- Strong clinical documentation skills and familiarity with billing/reporting processes
Education
- Bachelor's degree in Physical Therapy

- Patient evaluation and development of treatment plans
- Manual therapy and manual manipulation techniques
- Transfer assistance and safe patient handling
- Gait and balance assessment and intervention
- Clinical documentation best practices
- Billing/reporting procedures and Medicare familiarity
- In-service presentation and community education skills
- Ability to supervise and collaborate with support personnel
