Roles & Responsibilities
- Manage an active panel of SEARHC patients by coordinating care and follow-up
- Collaborate with providers and interdisciplinary team members to set individualized medical goals
- Coordinate multi-disciplinary care conferences for high-risk or complex patients
- Perform patient assessment, documentation, education, and necessary nursing interventions
- Track and manage referrals, waitlists, and outcomes across SEARHC and outside facilities
- Serve as primary link for case managers and discharge coordinators to ensure continuity of care
- Facilitate specialty clinic preparation and assist with procedures as needed
- Act as primary case manager for OB patients including intake, education, and postpartum follow-up
- Support telehealth visits by coordinating VTC connections and acting as nursing staff for those visits
- Participate in triage, emergency response, and ongoing development of the case management role
Qualifications
- Active, full, unrestricted Registered Nurse license in Alaska required at time of submittal
- Minimum 2 years of recent case management experience within the last 4 years required
- Alternative pathways accepted: at least 1 year general nursing with 6 months clinic experience at SEARHC OR 2 years general nursing with 1 year ambulatory or 1 year case management
- Basic Life Support (BLS) certification preferred
- National Case Management certification preferred
- Proof of flu vaccination and ability to meet annual infection control competencies
- Ability to complete fingerprint/background processing prior to start (fingerprint appointment and timely mailing required)
- Capacity to stand for long periods, lift/move patients or equipment, and work in a fast-paced clinical environment
Education
- Graduate of an accredited school of nursing required
- Bachelor's degree in Nursing preferred
- High school diploma or equivalent required
Knowledge & Skills
- Skilled in patient assessment, triage, and clinical documentation
- Strong care coordination, referral tracking, and follow-up skills
- Experience using EHR systems and reviewing EHR reminders and triggers
- Knowledge of safety and infection control principles
- Excellent oral and written communication and conflict resolution skills
- Ability to work independently and as part of a multi-disciplinary team
- Familiarity with telehealth (VTC) workflows and specialty clinic support
- Customer service focus and advocacy for patient-centered care
