Roles & Responsibilities
- Evaluate and treat skilled nursing facility patients using evidence-based SLP approaches
- Create individualized treatment plans and set meaningful, measurable goals
- Complete timely, thorough clinical documentation in Net Health per facility standards
- Collaborate with the interdisciplinary care team to coordinate patient-centered care
- Provide flexible coverage and float to a nearby sister facility as required
- Engage in mentorship and professional learning opportunities to strengthen clinical skills
Qualifications
- Active California SLP license (required)
- Skilled Nursing Facility (SNF) experience preferred; mentorship available if experience is limited
- Strong documentation skills and familiarity with Net Health is a plus
- Excellent time-management and ability to prioritize caseloads
- Collaborative mindset and ability to work well with interdisciplinary teams
- Willingness to float to a sister facility within walking distance
Knowledge & Skills
- Proficient in patient evaluation and treatment planning for geriatric/SNF populations
- Clinical documentation best practices and experience with Net Health
- Interdisciplinary team communication and care coordination
- Time management, caseload organization, and efficient workflow
- Skilled in evidence-based SLP interventions commonly used in SNF settings
