Roles & Responsibilities
- Provide skilled, patient-centered physical therapy interventions as directed by the supervising PT
- Assist patients with mobility, transfers, gait training, and functional activities to maximize independence
- Implement individualized treatment plans and document progress clearly and timely
- Collaborate with the interdisciplinary team to support discharge planning and continuity of care
- Educate patients and families on home programs, safety, and strategies to support recovery
- Maintain equipment, ensure a safe treatment environment, and adhere to facility policies
Qualifications
- Licensed Physical Therapist Assistant or eligible for state licensure
- Graduate of an accredited PTA program
- BLS/CPR certification required or willingness to obtain
- Previous experience in post-acute, rehab, or skilled nursing settings preferred
- Strong communication skills and ability to work collaboratively with a multidisciplinary team
- Current immunizations and compliance with facility onboarding requirements
Education
- Graduate of an accredited Physical Therapist Assistant program (Associate degree or equivalent)
Knowledge & Skills
- Skilled in therapeutic exercise, gait training, transfers, and use of adaptive equipment
- Familiarity with documentation standards and electronic medical records
- Ability to assess functional mobility and contribute to measurable treatment goals
- Excellent interpersonal skills for patient and family education
- Adaptable clinical judgment in a fast-paced post-acute environment
