Roles & Responsibilities

• Provide skilled, patient-centered physical therapy interventions as directed by the supervising PT
• Assist patients with mobility, transfers, gait training, and functional activities to maximize independence
• Implement individualized treatment plans and document progress clearly and timely
• Collaborate with the interdisciplinary team to support discharge planning and continuity of care
• Educate patients and families on home programs, safety, and strategies to support recovery
• Maintain equipment, ensure a safe treatment environment, and adhere to facility policies

Qualifications

• Licensed Physical Therapist Assistant or eligible for state licensure
• Graduate of an accredited PTA program
• BLS/CPR certification required or willingness to obtain
• Previous experience in post-acute, rehab, or skilled nursing settings preferred
• Strong communication skills and ability to work collaboratively with a multidisciplinary team
• Current immunizations and compliance with facility onboarding requirements

Education

• Graduate of an accredited Physical Therapist Assistant program (Associate degree or equivalent)

Knowledge & Skills

• Skilled in therapeutic exercise, gait training, transfers, and use of adaptive equipment
• Familiarity with documentation standards and electronic medical records
• Ability to assess functional mobility and contribute to measurable treatment goals
• Excellent interpersonal skills for patient and family education
• Adaptable clinical judgment in a fast-paced post-acute environment
