Roles & Responsibilities

• Provide thorough physical therapy evaluations and develop individualized treatment plans
• Deliver evidence-based interventions to improve mobility, strength, and functional independence
• Document patient progress accurately and communicate updates to the interdisciplinary team
• Coach patients and families on home exercise programs, safety, and prevention strategies
• Participate in discharge planning and coordinate care transitions to support optimal outcomes
• Collaborate with nursing and rehab staff to ensure high-quality, person-centered care

Qualifications

• Current, active California Physical Therapist license required
• Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy preferred
• BLS/CPR certification (American Heart Association or equivalent) preferred
• Experience in post-acute, inpatient rehabilitation, or similar settings is a plus
• Strong clinical reasoning, documentation skills, and ability to manage a diverse caseload
• Eligible to work in the U.S. or appropriate work authorization

Education

• Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy

Knowledge & Skills

• Skilled in patient assessment, mobility training, transfers, and gait training
• Familiarity with therapeutic modalities, manual therapy, and progressive exercise prescription
• Comfortable with electronic health record documentation and outcome tracking
• Excellent interpersonal and communication skills for patient education and team collaboration
• Time management and caseload prioritization to maintain quality and efficiency
