Roles & Responsibilities
- Evaluate patients and develop individualized occupational therapy treatment plans that promote independence
- Deliver targeted interventions to improve ADLs, functional mobility, and overall quality of life
- Collaborate with an interdisciplinary rehab team to coordinate care and optimize outcomes
- Educate patients and caregivers on safety, home programs, and adaptive techniques to support long-term gains
- Maintain accurate, timely documentation to ensure continuity of care and compliance
Qualifications
- Active California Occupational Therapist (OT) license required
- SNF or rehabilitation experience preferred
- Strong clinical reasoning and effective communication skills
- Ability to work independently while contributing as a collaborative team member
Knowledge & Skills
- Patient evaluation and individualized treatment planning
- ADLs improvement and functional mobility interventions
- Interdisciplinary collaboration with nursing and therapy team members
- Patient and caregiver education on safety and adaptive techniques
- Accurate clinical documentation and charting
- Strong communication, clinical reasoning, and teamwork
