Roles & Responsibilities
- Evaluate residents' functional abilities and create individualized occupational therapy plans
- Deliver patient-centered therapy to improve independence in daily living activities
- Collaborate with nursing, rehab, and family members to ensure cohesive care plans
- Document progress clearly and timely to support continuity of care and outcomes
- Train caregivers and staff on safe transfer techniques and adaptive strategies
- Measure and report functional outcomes to guide ongoing treatment decisions
Qualifications
- Current Occupational Therapy license for California (or compact/reciprocity as applicable)
- OTR (NBCOT) credential preferred
- Experience in skilled nursing or long-term care settings is highly valued
- Strong clinical reasoning, documentation, and patient/family communication skills
- Current CPR/BLS certification or willingness to obtain promptly
- Clear background check and ability to meet facility onboarding requirements
Education
- Master's or Doctorate in Occupational Therapy preferred; Bachelor's with appropriate credentialing accepted where applicable
- Completion of accredited OT program and NBCOT eligibility or certification
Knowledge & Skills
- Skilled in ADL/IADL assessment and adaptive equipment recommendations
- Knowledge of Medicare/Medicaid documentation and skilled nursing workflows
- Able to develop measurable short- and long-term goals tied to functional outcomes
- Good time management, interdisciplinary communication, and problem solving
- Comfortable with electronic medical records and timely clinical documentation
