Roles & Responsibilities
- Evaluate patients in skilled nursing to determine occupational performance and functional needs
- Develop individualized treatment plans that target ADLs, mobility, and daily independence
- Deliver hands-on therapy and adaptive techniques to maximize patient safety and quality of life
- Collaborate with nursing, PT, SLP, and interdisciplinary teams to coordinate care
- Train patients, families, and staff on adaptive equipment and home strategies
- Document progress clearly and timely to support continuity of care and discharge planning
- Advocate for patient-centered goals and support smooth transitions of care
Qualifications
- Active California Occupational Therapy license (required)
- OTR/L credential preferred
- Experience in skilled nursing or long-term care settings preferred
- Current Basic Life Support (BLS) certification or willingness to obtain
- Strong clinical reasoning, assessment, and documentation skills
- Excellent communication and teamwork mindset; compassionate approach to patient care
Education
- Master’s or Doctorate in Occupational Therapy from an accredited program
- Completion of a professional OT program and eligibility for state licensure
Knowledge & Skills
- Proficiency with ADL and functional assessments
- Skilled in adaptive equipment recommendations and therapeutic activity design
- Knowledge of safe patient handling and fall prevention strategies
- Familiarity with documentation standards for skilled nursing and regulatory requirements
- Good time management, prioritization, and the ability to work within interdisciplinary teams
- Comfort teaching caregivers and family members practical techniques
