Roles & Responsibilities
- Assess residents' functional status and design individualized occupational therapy plans
- Develop meaningful, goal-driven treatment plans that help residents regain independence
- Implement therapeutic activities for ADLs, mobility, and adaptive equipment use
- Collaborate with nursing, PT, SLP and social work to coordinate holistic care
- Document progress clearly and timely in the medical record and on care plans
- Educate residents and caregivers on strategies, safety and home programs
- Adapt interventions creatively to meet changing clinical and cognitive needs
Qualifications
- Licensed Occupational Therapist (OTR/L) eligible to practice in California
- Current BLS/CPR certification or willingness to obtain upon hire
- Experience working in skilled nursing or long-term care settings preferred
- Strong clinical reasoning, assessment and documentation skills
- Excellent interpersonal and communication skills with residents and families
- Ability to work collaboratively in an interdisciplinary team
Education
- Degree in Occupational Therapy (BS/MS/OTD) from an accredited program
Knowledge & Skills
- Skilled in ADL assessment and adaptive equipment recommendations
- Knowledge of rehab techniques for older adults and medically complex patients
- Experience with care planning, discharge planning and progress documentation
- Comfortable using electronic medical records and charting systems
- Ability to train and educate nursing staff and caregivers on safe transfers and strategies
