Roles & Responsibilities

• Assess and manage patients in the ED using InterQual Criteria to determine appropriate level of care
• Coordinate care across the continuum by collaborating with physicians, social work, admissions, and community resources
• Facilitate safe and efficient patient throughput, including admission status recommendations and outpatient disposition planning
• Provide targeted care coordination for patients with mental health needs, frequent utilization, chronic pain, and abuse
• Develop and participate in creation of clinical practice guidelines and physician order sets to improve quality and reduce costs
• Lead discharge planning and arrange referrals to all levels of care to support safe transitions
• Document interventions and care plans accurately in the EMR and communicate effectively with multidisciplinary teams

Qualifications

• Active, current, full and unrestricted RN license in any US state, territory, or Puerto Rico
• Associate Degree in Nursing (required)
• Basic Life Support (BLS) certification by the American Heart Association (current)
• Obtain Case Manager Certification (CCM or ACM) within 2 years if not already certified
• Minimum 2 years of current care/case management experience in the Emergency Department
• Minimum 2 years patient care experience (hospital, home health, or skilled nursing)
• At least 1 year of travel nursing experience
• Fingerprinting completed and cleared prior to start

Education

• Associate Degree in Nursing

Knowledge & Skills

• Proficient with Electronic Medical Records and documentation best practices
• Comfortable using Microsoft Outlook, Word, and Excel for communication and reporting
• Skilled in patient assessment, care coordination, and discharge planning
• Experience applying InterQual Criteria for patient status determination
• Strong interdisciplinary collaboration and resource management capabilities
• Ability to work effectively with patients across the lifespan, including those with mental health needs
• Familiarity with developing clinical practice guidelines and physician order set input
