Shift Type: Sunday-Saturday (variable), 7:00 AM - 5:30 PM, weekend rotation with on-call, Day shift
Hours per Week: 42
Candidate Pay Rate: $74 per hour

Roles & Responsibilities
- Assess and manage patients in the ED using InterQual Criteria to determine appropriate level of care
- Coordinate care across the continuum by collaborating with physicians, social work, admissions, and community resources
- Facilitate safe and efficient patient throughput, including admission status recommendations and outpatient disposition planning
- Provide targeted care coordination for patients with mental health needs, frequent utilization, chronic pain, and abuse
- Develop and participate in creation of clinical practice guidelines and physician order sets to improve quality and reduce costs
- Lead discharge planning and arrange referrals to all levels of care to support safe transitions
- Document interventions and care plans accurately in the EMR and communicate effectively with multidisciplinary teams
Qualifications
- Active, current, full and unrestricted RN license in any US state, territory, or Puerto Rico
- Associate Degree in Nursing (required)
- Basic Life Support (BLS) certification by the American Heart Association (current)
- Obtain Case Manager Certification (CCM or ACM) within 2 years if not already certified
- Minimum 2 years of current care/case management experience in the Emergency Department
- Minimum 2 years patient care experience (hospital, home health, or skilled nursing)
- At least 1 year of travel nursing experience
- Fingerprinting completed and cleared prior to start
Education
- Associate Degree in Nursing
Knowledge & Skills
- Proficient with Electronic Medical Records and documentation best practices
- Comfortable using Microsoft Outlook, Word, and Excel for communication and reporting
- Skilled in patient assessment, care coordination, and discharge planning
- Experience applying InterQual Criteria for patient status determination
- Strong interdisciplinary collaboration and resource management capabilities
- Ability to work effectively with patients across the lifespan, including those with mental health needs
- Familiarity with developing clinical practice guidelines and physician order set input
