Roles & Responsibilities

• Provide hands-on therapeutic care and assist with rehabilitation plans to help patients regain independence
• Implement individualized treatment plans under PT direction and monitor patient progress
• Document interventions and patient responses clearly and timely in the medical record
• Collaborate with an interdisciplinary team to coordinate safe, effective care
• Educate patients and families on exercises, home programs, and safety strategies
• Contribute to discharge planning and transitions of care to support positive outcomes
• Maintain a safe treatment environment and follow infection control and facility protocols

Qualifications

• Licensed Physical Therapist Assistant in New Hampshire or eligible for licensure
• Current Basic Life Support (BLS) certification (or willingness to obtain promptly)
• Experience in physical therapy or rehabilitation settings preferred
• Strong communication skills and comfort working within an interdisciplinary team
• Ability to follow care plans, document accurately, and prioritize patient safety

Education

• Associate degree or accredited PTA program for Physical Therapist Assistant
• For those functioning in PT roles: entry-level degree in physical therapy (as applicable)

Knowledge & Skills

• Skilled in therapeutic exercise, gait training, and functional mobility interventions
• Knowledge of rehabilitation principles and common modalities used in post-acute care
• Competent documentation skills in electronic health records
• Ability to assess patient progress and report changes to supervising PT
• Strong patient education and motivational coaching abilities
• Good time management, organization, and teamwork mindset
