Roles & Responsibilities
- Provide hands-on therapeutic care and assist with rehabilitation plans to help patients regain independence
- Implement individualized treatment plans under PT direction and monitor patient progress
- Document interventions and patient responses clearly and timely in the medical record
- Collaborate with an interdisciplinary team to coordinate safe, effective care
- Educate patients and families on exercises, home programs, and safety strategies
- Contribute to discharge planning and transitions of care to support positive outcomes
- Maintain a safe treatment environment and follow infection control and facility protocols
Qualifications
- Licensed Physical Therapist Assistant in New Hampshire or eligible for licensure
- Current Basic Life Support (BLS) certification (or willingness to obtain promptly)
- Experience in physical therapy or rehabilitation settings preferred
- Strong communication skills and comfort working within an interdisciplinary team
- Ability to follow care plans, document accurately, and prioritize patient safety
Education
- Associate degree or accredited PTA program for Physical Therapist Assistant
- For those functioning in PT roles: entry-level degree in physical therapy (as applicable)
Knowledge & Skills
- Skilled in therapeutic exercise, gait training, and functional mobility interventions
- Knowledge of rehabilitation principles and common modalities used in post-acute care
- Competent documentation skills in electronic health records
- Ability to assess patient progress and report changes to supervising PT
- Strong patient education and motivational coaching abilities
- Good time management, organization, and teamwork mindset
