Roles & Responsibilities

• Provide compassionate, resident-centered nursing care throughout your shift
• Administer medications and treatments safely and accurately
• Monitor and document vital signs, patient status, and changes in condition
• Implement care plans and collaborate with the interdisciplinary team to meet resident goals
• Perform routine clinical procedures and wound care as needed
• Maintain accurate, timely electronic health record documentation
• Communicate clearly with residents, families, and staff to support continuity of care
• Adhere to facility policies, infection control practices, and safety protocols

Qualifications

• Current Licensed Practical Nurse (LPN) license — Vermont license preferred or compact-eligible where applicable
• Active CPR/BLS certification required
• Previous experience in long-term care, skilled nursing, or similar setting preferred
• Ability to work evenings and every other weekend with a positive attitude
• Up-to-date immunizations and ability to pass background and health screenings
• Excellent communication skills and a collaborative approach to patient care

Education

• Completion of an accredited LPN/LVN program or equivalent practical nursing education

Knowledge & Skills

• Solid knowledge of medication administration and documentation standards
• Competence in vital sign assessment and basic clinical monitoring
• Familiarity with electronic health records and charting best practices
• Strong infection control knowledge and adherence to safety protocols
• Good time management and prioritization in a fast-paced care environment
• Empathy, teamwork, and effective communication with residents and families
