Roles & Responsibilities
- Conduct thorough physical therapy evaluations and create individualized treatment plans
- Deliver hands-on therapy, therapeutic exercise, and modality-based interventions
- Document patient progress clearly and timely in the medical record
- Collaborate with nurses, physicians, and the rehab team to optimize outcomes
- Educate patients and caregivers on home programs, safety, and discharge planning
- Contribute to a positive, patient-centered environment and quality improvement efforts
Qualifications
- Current California Physical Therapist license or eligible to obtain one
- Basic Life Support (BLS) / CPR certification required
- Prior experience in outpatient, post-acute, or contract settings preferred
- Strong clinical judgment, communication skills, and a patient-first attitude
- Ability to pass facility onboarding and background checks
Education
- Doctor of Physical Therapy (DPT) preferred; Bachelor's or Master's in Physical Therapy accepted
Knowledge & Skills
- Skilled in musculoskeletal assessment, manual therapy, and therapeutic exercise
- Comfortable with electronic medical records and timely documentation
- Strong interpersonal skills for coaching patients and working with interdisciplinary teams
- Familiarity with rehab modalities, mobility training, and discharge planning
