Roles & Responsibilities
- Provide compassionate, patient-centered nursing care within the LPN scope of practice
- Administer medications and treatments accurately and timely
- Monitor patient status and communicate changes to the care team
- Document patient assessments, interventions, and responses clearly and professionally
- Collaborate with RNs, providers, and support staff to deliver coordinated care
- Support patients and families with education, comfort measures, and emotional support
Qualifications
- Current LPN license (active and in good standing) — NH licensure preferred
- Minimum experience: comfortable working in long-term care or similar settings (exact years not specified)
- Basic Life Support (BLS) certification or willingness to obtain
- Up-to-date immunizations and ability to pass background and drug screening
- Strong communication skills and a team-first attitude
- Ability to handle clinical documentation and medication administration reliably
Education
- LPN diploma or practical nursing certificate from an accredited program
- High school diploma or GED
Knowledge & Skills
- Clinical assessment and monitoring skills appropriate for LPN practice
- Medication administration and documentation best practices
- Infection control and safety protocols
- Basic wound care and patient hygiene assistance
- Time management and prioritization in a busy care environment
- Collaborative communication with interdisciplinary teams
