Roles & Responsibilities
- Administer medications and treatments accurately and on schedule, following facility policies
- Monitor, assess and document patient vitals and condition changes, escalating concerns to the RN or provider
- Provide compassionate basic nursing care that promotes patient dignity and comfort
- Collaborate with RNs, CNAs and the interdisciplinary team to coordinate excellent patient-centered care
- Maintain clear, timely and accurate charting in the electronic health record
- Support admissions, discharges and safe patient transfers to ensure smooth transitions
- Follow infection control protocols and contribute to a safe environment for patients and staff
Qualifications
- Current LPN license (required)
- Able and willing to work nights and fulfill weekend commitments as listed
- Own reliable transportation (required for this assignment)
- Comfortable working in a fast-paced long-term care/rehab setting
- Professional demeanor, strong communication skills, and a team-oriented attitude
Education
- Graduate of an accredited Practical Nursing program (LPN diploma or certificate)
Knowledge & Skills
- Medication administration and basic clinical assessments
- Proficient in taking and interpreting vital signs
- Skilled in documentation and charting best practices
- Basic wound care and assistance with ADLs
- Strong patient communication, empathy, and teamwork
