Roles & Responsibilities

• Provide skilled physical therapy visits in patients’ homes to promote independence and recovery
• Perform thorough evaluations and develop individualized care plans using best-practice interventions
• Complete accurate OASIS documentation and other required clinical records in a timely manner
• Coordinate with interdisciplinary team members including nursing, OT, SLP and social work to optimize patient outcomes
• Educate patients and families on safe mobility, fall prevention, home exercise programs and adaptive strategies
• Monitor progress, adjust plans of care, and communicate changes to the care team and referring clinicians
• Represent the agency professionally in the community and maintain timely, reliable travel between patient homes

Qualifications

• Active New Hampshire physical therapy license (Compact allowed)
• Minimum 2 years of recent physical therapy experience in home health (required)
• Current American Heart Association BLS certification
• OASIS documentation experience (required)
• Experience with patients from pediatrics through geriatrics
• Comfortable driving your own vehicle in winter conditions and able to travel within a large rural territory
• One supervisory signed reference from the past year and a current PT skills checklist (as requested for submission)
• Permanent address included with submission and ability to meet the NO LOCALS requirement (50+ miles from facility)

Education

• Degree in Physical Therapy (e.g., DPT) or equivalent required for licensure

Knowledge & Skills

• Strong home health clinical skills and safety-focused treatment planning
• Proficiency with OASIS documentation and home health regulatory expectations
• Good clinical reasoning across a wide range of ages and diagnoses
• Clear communication with patients, caregivers and the interdisciplinary team
• Time management and route planning for efficient in-home visits
• Adaptability to provide care in varied home environments and weather conditions
