Roles & Responsibilities
- Administer medications and treatments safely and accurately according to facility policies
- Monitor and document resident vital signs and condition changes, escalating concerns promptly
- Provide compassionate assistance with activities of daily living to promote dignity and independence
- Support wound care, catheter care, and other nursing procedures as directed
- Collaborate with RNs, therapists, and interdisciplinary team members to deliver coordinated care
- Maintain clear, timely, and professional documentation in the electronic medical record
- Adhere to infection control standards and facility safety protocols at all times
Qualifications
- Active Licensed Practical Nurse (LPN) license in good standing (or eligible for New Mexico licensure)
- Basic Life Support (BLS/CPR) certification — current or willing to obtain
- Comfortable working independently during night shifts and escalating issues appropriately
- Strong communication and teamwork skills with a patient-centered approach
- Previous experience in long-term care, skilled nursing, or rehabilitation is preferred
Education
- Graduate of an accredited practical nursing program or equivalent LPN training
Knowledge & Skills
- Medication administration and documentation best practices
- Clinical assessment skills: vital signs, basic wound assessment, and recognizing changes in condition
- Familiarity with infection control, safety protocols, and resident rights
- Proficiency with electronic medical records and clear clinical charting
- Good time management and prioritization during busy night shifts
