Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in accordance with care plans and facility policies
- Administer medications and treatments safely, accurately documenting each intervention
- Monitor vital signs and report significant changes promptly to the care team
- Assist with daily living activities while promoting resident dignity and independence
- Collaborate with nurses, therapists, and providers to support optimal resident outcomes
- Document assessments, interventions, and resident responses in the electronic health record clearly and timely
- Support admission, transfer, and discharge activities to ensure smooth transitions of care
- Participate in quality improvement initiatives and maintain a safe infection-control environment
Qualifications
- Current LPN license in the state of practice or eligible for New Hampshire licensure
- Active Basic Life Support (BLS) certification (or willingness to obtain prior to start)
- Recent LPN experience in long-term care, skilled nursing, or similar clinical setting preferred
- Ability to pass facility-required background checks and health screenings
- Strong interpersonal skills and a team-first attitude
Education
- LPN diploma or certificate from an accredited nursing program
- High school diploma or equivalent
Knowledge & Skills
- Skilled in medication administration, basic wound care, and resident assessments
- Comfortable with electronic health record charting and documentation
- Knowledge of infection control practices and patient safety principles
- Excellent communication skills with residents, families, and interdisciplinary teams
- Time management and prioritization to handle a busy clinical workload
- Ability to remain calm and effective in clinically urgent situations
