Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in accordance with care plans and facility policies
• Administer medications and treatments safely, accurately documenting each intervention
• Monitor vital signs and report significant changes promptly to the care team
• Assist with daily living activities while promoting resident dignity and independence
• Collaborate with nurses, therapists, and providers to support optimal resident outcomes
• Document assessments, interventions, and resident responses in the electronic health record clearly and timely
• Support admission, transfer, and discharge activities to ensure smooth transitions of care
• Participate in quality improvement initiatives and maintain a safe infection-control environment

Qualifications

• Current LPN license in the state of practice or eligible for New Hampshire licensure
• Active Basic Life Support (BLS) certification (or willingness to obtain prior to start)
• Recent LPN experience in long-term care, skilled nursing, or similar clinical setting preferred
• Ability to pass facility-required background checks and health screenings
• Strong interpersonal skills and a team-first attitude

Education

• LPN diploma or certificate from an accredited nursing program
• High school diploma or equivalent

Knowledge & Skills

• Skilled in medication administration, basic wound care, and resident assessments
• Comfortable with electronic health record charting and documentation
• Knowledge of infection control practices and patient safety principles
• Excellent communication skills with residents, families, and interdisciplinary teams
• Time management and prioritization to handle a busy clinical workload
• Ability to remain calm and effective in clinically urgent situations
