Roles & Responsibilities

• Assess patients’ functional abilities and occupational needs to create meaningful, individualized plans of care
• Develop and implement therapeutic interventions that promote independence and safety
• Document patient progress and outcomes clearly and timely in the medical record
• Collaborate with nursing, rehabilitation, and support staff to coordinate patient-centered care
• Educate patients and families on techniques, adaptive equipment, and strategies to support daily living
• Measure and report treatment effectiveness, adjusting plans to meet evolving goals
• Participate in team meetings and discharge planning to ensure smooth transitions of care

Qualifications

• Occupational Therapy (OT) credential/registration (OT) — core skill for this role
• Experience working in post-acute, rehab, or similar settings is a strong fit
• Strong clinical reasoning and documentation skills
• Excellent communication and teamwork abilities
• Available for the contract assignment dates (starts 2026-01-26 through 2026-04-25)

Knowledge & Skills

• Skilled in patient assessment and functional goal setting
• Knowledge of adaptive equipment and therapeutic modalities
• Competent with clinical documentation and care planning
• Able to teach and train patients and caregivers effectively
• Good organizational skills and ability to collaborate across disciplines
