Roles & Responsibilities
- Assess patients’ functional abilities and occupational needs to create meaningful, individualized plans of care
- Develop and implement therapeutic interventions that promote independence and safety
- Document patient progress and outcomes clearly and timely in the medical record
- Collaborate with nursing, rehabilitation, and support staff to coordinate patient-centered care
- Educate patients and families on techniques, adaptive equipment, and strategies to support daily living
- Measure and report treatment effectiveness, adjusting plans to meet evolving goals
- Participate in team meetings and discharge planning to ensure smooth transitions of care
Qualifications
- Occupational Therapy (OT) credential/registration (OT) — core skill for this role
- Experience working in post-acute, rehab, or similar settings is a strong fit
- Strong clinical reasoning and documentation skills
- Excellent communication and teamwork abilities
- Available for the contract assignment dates (starts 2026-01-26 through 2026-04-25)
Knowledge & Skills
- Skilled in patient assessment and functional goal setting
- Knowledge of adaptive equipment and therapeutic modalities
- Competent with clinical documentation and care planning
- Able to teach and train patients and caregivers effectively
- Good organizational skills and ability to collaborate across disciplines
