Roles & Responsibilities

• Assess and evaluate patients to develop individualized treatment plans
• Deliver skilled physical therapy interventions to improve mobility and function
• Document patient progress accurately and communicate updates to the care team
• Collaborate with interdisciplinary team members to coordinate comprehensive care
• Educate patients and families on exercises, safe mobility, and home programs
• Participate in discharge planning and ensure continuity of care
• Contribute to quality improvement initiatives and maintain regulatory compliance

Qualifications

• Licensed Physical Therapist in New Hampshire or ability to obtain state licensure promptly
• Current CPR/BLS certification (or willing to obtain upon hire)
• Strong clinical reasoning and hands-on treatment skills in physical therapy
• Comfortable working with adult and geriatric populations in a rehab setting
• Excellent communication and teamwork skills; patient-centered and professional

Education

• Doctor of Physical Therapy (DPT) preferred or Master of Physical Therapy

Knowledge & Skills

• Skilled in assessment, manual therapy techniques, therapeutic exercise, and gait training
• Knowledge of rehab documentation, outcomes measurement, and care planning
• Ability to prioritize caseload and manage time effectively
• Familiarity with safety protocols, fall prevention, and mobility equipment
• Comfortable using electronic medical records and following facility policies
