Roles & Responsibilities

• Evaluate patients to determine functional limitations, strengths, and therapy goals
• Develop and implement individualized treatment plans using evidence-based techniques
• Deliver therapeutic interventions such as gait training, therapeutic exercise, and manual therapy
• Document skilled services accurately and timely in the facility’s medical record system
• Collaborate with nurses, physicians, and multidisciplinary team members to coordinate care
• Educate patients and families on home programs, safety, and strategies to maintain gains
• Participate in team meetings and contribute to quality improvement and patient outcomes

Qualifications

• Active NH license as a Physical Therapist or Physical Therapist Assistant (or eligible to obtain)
• Graduation from an accredited PT or PTA program appropriate to the role
• Current BLS/CPR certification (or willing to obtain prior to start)
• Ability to pass facility onboarding and background checks in a timely manner
• Strong communication skills and patient-centered approach
• Previous experience in outpatient or post-acute rehab is a plus

Education

• Physical Therapist: Doctor of Physical Therapy (DPT) or equivalent clinical degree
• Physical Therapist Assistant: Associate degree from an accredited PTA program

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, and mobility training
• Familiarity with documentation standards and electronic health record systems
• Knowledge of safety protocols, infection control, and rehab equipment
• Ability to deliver compassionate patient and family education
• Good teamwork, time management, and clinical decision-making skills
