Roles & Responsibilities
- Assess patients’ functional abilities and needs to create meaningful goals
- Develop and implement individualized treatment plans that promote independence
- Deliver hands-on therapeutic interventions and adaptive strategies
- Document patient progress clearly and timely to support outcomes and continuity of care
- Collaborate with nurses, therapists, and family members to coordinate care plans
- Educate patients and caregivers on home programs, adaptive equipment, and safety
- Participate in discharge planning to ensure safe, effective transitions
Qualifications
- Class: OT or COTA — this posting supports Occupational Therapist and Certified Occupational Therapy Assistant candidates
- Active Occupational Therapist (OT) license or Certified Occupational Therapy Assistant (COTA) credential — or eligible for Maine licensure
- Current BLS/CPR certification preferred
- Comfortable working in a post-acute/rehabilitation setting (experience preferred)
- Able to complete required background checks, health screenings, and onboarding paperwork
- Strong communication skills and a collaborative mindset
Education
- Bachelor’s degree in Occupational Therapy (for OT)
- Associate degree or completion of an accredited OTA program (for COTA)
Knowledge & Skills
- Skilled in functional assessments and standardized OT evaluation tools
- Knowledge of adaptive equipment, orthotics, and durable medical equipment recommendations
- Ability to create goal-driven treatment plans and measure outcomes
- Excellent documentation skills and familiarity with clinical charting
- Strong interpersonal skills for patient and family education
- Capacity to work effectively within an interdisciplinary care team
