Roles & Responsibilities

• Assess patients’ physical function and set measurable rehabilitation goals
• Develop and implement individualized treatment plans using evidence-based techniques
• Deliver therapeutic interventions to improve mobility, strength, and independence
• Document patient progress thoroughly and communicate updates with the care team
• Collaborate with nurses, physicians, and rehab staff to optimize patient outcomes
• Educate patients and families on home programs, safety, and fall prevention
• Supervise and mentor support staff and therapy assistants as needed

Qualifications

• Licensed Physical Therapist in Pennsylvania, or eligible to obtain state licensure
• Degree from an accredited Physical Therapy program (DPT preferred)
• Current CPR/BLS certification or willingness to obtain upon hire
• Comfortable with outpatient/contract therapy settings and a 13-week assignment
• Strong clinical assessment, treatment planning, and documentation skills
• Professional, team-oriented mindset with excellent communication skills

Education

• Doctor of Physical Therapy (DPT) preferred; completion of an accredited PT program required

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy, and mobility training
• Proficient in patient assessment, outcome measurement, and goal setting
• Familiar with documentation standards and electronic medical records
• Ability to prioritize caseloads and manage time effectively
• Strong interpersonal skills for patient/family education and team collaboration
